The Medical Model
We know that everyone's needs are different, which is why our care plans are tailor-made for everyone. We offer personalized care plans, annual health assessments, first aid care, medication administration, oxygen therapy, diabetic management and restorative therapy.
Medical care is a cornerstone of our services. AADC offers daily health monitoring, including vital sign checks, medication administration, and assistance with activities of daily living (ADLs) such as eating, bathing, and mobility. Our trained staff is equipped to handle a wide range of medical conditions, ensuring participants receive consistent and professional care.
Levels of care
Tier 1: Lowest Level of Care
- Independent member may need some assistive care
- Able to walk or get around in wheelchair per self
- Able to toilet and feed self
Tier 2: Moderate Level of Care
- May require standby or 1 hand-on assistance.
- Will need meal set up and limited assit with toileting
- Oxygen as needed per MD
Tier 3: Highest Level of Care
- Require stand-up lift for transfers
- Need assist with toileting and feeding
- Oxygen required contiouously per MD (4 Liters max)
Transportation
- Non-Medical Transportation within a 20 mile radius of the center
- To/From facility for no charge per request
- Travel to doctor appointments per approval
